All new clients are required to have an Initial Consultation with one of our OT’s

A Comprehensive Assessment is completed on each new client prior to the start of a treatment regime, and a report will be provided accordingly  

Previous OT reports will be considered only if they are less than 6 months old and may need additional assessments for therapy to commence at SSOT 

NDIS Equipment or Home Mods Assessments require 3 months notice  

NDIS Review reports require 3 months notice and additional assessments 

Assessments and Reports are charged for at the current  

NDIS rate per hour for the time taken to complete the assessments, score the results and write the report.
