Assessments & Reports Information

New Clients

All new clients (Parents) are required to have an Initial Consultation with one of our OT’s.

Diagnostic Assessment:

A Comprehensive Assessment is completed on each new client prior to the start of a treatment plan, or to contribute information towards a possible diagnosis - a Report will be provided and charged for according to the time taken to complete it.

Clients that have previously had an OT Assessment (within the past 6 months):

For new clients that have previously had an OT assessment elsewhere, please supply a copy of the OT report at your Initial Consultation at SSOT.  This will allow us to consider the appropriate inclusion of your child into our group programs. Please understand that additional assessment recommendations may be made following observations within the group. 

NDIS Progress Review Reports

Require a Review Assessment and a booking 3 months in advance, irrespective of the duration of the current NDIS plan.

NDIS AT Reports

Require 3 -10 hours in assessment and preparation, and are required to be booked a minimum of 3 months in advance of NDIS review meetings.

Amendments to reports or “Letters of Support are also charged according to the time taken to amend the report, or draft the letter with new information.

All services, including assessments and reports are charged at the current NDIS rate per hour, for the time taken to complete the assessments, follow-up with relevant parties, score the results and write the report.


NDIS AT Equipment or Home Mods Assessments - require 3 months prior notice  

NDIS Review Reports - require Review Assessments and 3 months notice
